A. In April 2003, it was decided that the issuing company is to handle application proceedings for lost registrations as a substitute for public summons and judgment of exclusion of shareholders' rights that had been the previously established methods.
Therefore, you will need to go through the application proceedings for lost registration if you lost your stock certificate. As for the necessary documents and details of the proceedings, please contact Mitsubishi UFJ Trust and Banking Corporation, our transfer agent.
* A new stock certificate will be issued when there is no claim from an owner of the lost certificate for one (1) year from the day following the registration.
A. Please obtain the form titled "Request for Negotiation on Odd-Lot Stock" from Mitsubishi UFJ Trust and Banking Corporation, our transfer agent, and submit the following documents:

"Request for Negotiation on Odd-Lot Stock"
"Stock Certificate" (Not required when it is not issued)
* Cancellation after the request of negotiation is not accepted. If you use the custody and depositary system of stock certificates, please go though the necessary proceedings at the securities company to which you deposit your stock.
A. The deemed acquisition value, being 80% of the closing price as of October 1, 2001, is as follows:
